Effective Autumn Quarter 2007-08, the process of review for undergraduate petitions has changed. Petitions related to academic standing and policies are now administered by the Office of the Vice Provost for Undergraduate Education (VPUE), Sweet Hall, 1st floor.

Forms - Coterm

Effective Autumn Quarter 2008-09, forms related to coterminal students are handled by the Office of the Vice Provost for Undergraduate Education (VPUE), Sweet Hall, 1st floor.

 

The following petitions and procedures are administered by the Office of the University Registrar. Students should drop off forms at the Student Services Center, Tresidder Union, Second Floor.

Transfer Credit

Request for Transfer Credit Evaluation Form (pdf)

Use this forms if you require pre-approval for future transfer credit or if you want to petition to transfer course work already completed.

Enrollment

Leave of Absence Form (undergraduate) (pdf)

A leave of absence is required for any term of the Academic Year (Summer terms excluded) for which a student does not wish to enroll in classes. For students with multiple degree programs or majors, note that leaves of absence are only granted for all programs and majors. Leaves are not be granted for more than one year at a time. A leave of absence from your program may not exceed a cumulative total of two years. Permit to Attend for Services Only (PSO) (pdf)

Undergraduates in their terminal quarter who are completing an honors thesis or clearing incompletes and require registered student status may petition, on a one-time basis, for Permit to Attend for Services Only (PSO) registration status.

Permit to Attend (PTA) (pdf)

Undergraduates who require registered student status and are not enrolled in courses may petition, on a one-time basis, for Permit to Attend (PTA) registration status.

13th-Quarter Petition (pdf)

Undergraduates who have completed at least 12 quarters of full-time tuition may petition to register at a reduced tuition (minimum 8 units) for their final undergraduate quarter at Stanford.

Graduation Quarter Petition (pdf)

Intended primarily for undergraduates in their terminal quarter who have completed all course work and degree requirements, and who are not using Stanford resources.

Annulment of Summer Registration (pdf)

Use this form to withdraw from all of your summer courses and receive any applicable refund amounts, if eligible.

Undergraduate Study and Programs

Declaration or Change of Undergraduate Major, Minor, Honors, or Degree Program (pdf)

Prior to applying to graduate, declare or make changes to majors, minors, honors, and degree programs through Axess. After applying to graduate or after the application to graduate deadline, use this form.

Changes to GER Requirements (effective Autumn Quarter 2005-06) (pdf)

Overview of the changes to the General Education Requirments effective Autumn Quarter 2005-06.

Switch to New GER Requirements (Effective Autumn Quarter 2005-06) (pdf)

This form is for undergraduates admitted Autumn Quarter 2003-04 or before who would like to switch to the new General Education requirements that became effective Autumn Quarter 2005-06.

Last Units Out of Residence (pdf)

This form is for undergraduates who wish to petition to take their final 15 units out of residence to complete their degree requirements.

Major-Minor and Multiple Major Course Approval Form (pdf)

Use this form to certify that plan of study meets all requirements for the major(s) and any minor(s). Requires departmental signatures and is submitted to the Office of the University Registrar.

Forms - Coterminal


Procedures and Application Forms for Coterminal Students

This packet reflect changes made to the coterminal degree program adopted by the Faculty Senate in November 2003. The Coterminal Degree Program permits Stanford undergraduates to study for a bachelor's and a master's degree simultaneously, in the same or different departments.

Coterminal Course Approval Form

Coterm students can use this form to request that courses be transferred from one career to the other.

Admissions and Reinstatement

Application for Reinstatement in Graduate Study (pdf)

If you are an admitted graduate student who has not maintained continuous registration (or been on an approved leave of absence), you must apply for reinstatement. After completing this form, submit it to your department for approval and then submit it to the Graduate Admissions Office with the appropriate fee.

Recommendation Form for Graduate Study (pdf)

Part of the application process, should be mailed directly to the department.

Financial Resources Certification Form (pdf)

Form for documenting dependents and sources of funding.

Graduate Study and Programs

Program Proposal for a Master's Degree (pdf)

Proposal form for M.A., M.F.A., M.A.T., or M.S. students.

Graduate Program Authorization Petition (pdf)

Form for changing or adding a graduate degree program.

Application for Ph.D. Minor (pdf)

Use this form to add a minor to an existing Ph.D. program of study.

Request for Terminal Graduate Registration (TGR) Status (pdf)

TGR allows students to register at a reduced tuition rate while working on a dissertation, thesis, or department project. If you are eligible, this form must be completed and submitted to your department for approval.

Application for Graduate Residency Credit (pdf)

Use this form to transfer a maximum of 45 eligible units into your Ph.D., Ed.D., D.M.A., Ed.S., or Engineer program for graduate work done at another institution.

Switch to Unit-Based Residency Requirement

Use this form to switch from a term-based to a unit-based residency requirement.

Enrollment and Tuition

Request for Graduate Tuition Adjustment (pdf)

Submit this petition to request for one-time only tuition reduction during final quarter of degree program.

Enrollment Agreement for Students with Multiple Programs (pdf)

Use this agreement to determine the tuition amount assessed each quarter for students enrolled in multiple degree programs with different tuition rates.

Graduate Section Change Form (pdf)

This form must be submitted by a graduate or coterminal student who wishes to change sections of a course in which they are currently enrolled after the add deadline. Before the add deadline all section changes must be made in Axess.

Petition to Change Course Enrollment (Graduate Students) (pdf)

Graduate or coterminal students should use this form to add course/s after the deadline, change unit total of course/s after the deadline, extend an Incomplete after the deadline, or repeat a course for a second time. Changes requested after the posted academic deadlines must demonstrate exceptional circumstances as they are an exception to Faculty Senate course enrollment policies. Before the deadlines, all enrollment changes must be made via Axess.

Petition to Withdraw from a Course (Graduate Students) (pdf)

Graduate or coterminal students should use this form to withdraw from a course after the deadline. Changes after the posted academic deadlines must demonstrate exceptional circumstances as they are an exception to Faculty Senate course enrollment policies. Before the withdraw deadline, all course withdraws must be made via Axess.

Leave of Absence Form (pdf)

Returning Graduate Student Request to Register (pdf)

Graduate students returning early from an approved leave of absence must file this form before the beginning of the quarter in which they wish to return.

Request to Permanently Withdraw from Degree Program (pdf)

Use this form to permanently withdraw from your requested degree program on the date indicated. You are acknowledging that should you decide to return to the indicated degree program in the future, you will have to apply for reinstatement and pay the necessary reinstatement fee(s).

Graduation Quarter Petition (pdf)

Intended primarily for graduate students who have completed all course work, degree requirements, and residency requirements.

Application for Exchange Program at UC Berkeley or UC San Francisco (pdf)

This application is only for Stanford graduate students who wish to enroll in University of California, Berkeley (UC Berkeley) or University of California, San Francisco (UCSF) courses.

Candidacy

Application for Extension of Candidacy or Master's Program (pdf)

If your candidacy or master's program has expired, this form should be completed and submitted to your department for approval.

Application for Candidacy for Degree of Engineer (pdf)

Must be submitted by candidates for Engineer degree by the end of the second quarter in the program (fourth quarter for Honors Cooperative students).

Application for Candidacy for Doctoral Degree (pdf)

Students are expected to be admitted for candidacy by the end of the second year of doctoral study. This form should be completed and submitted to your department for approval.

Doctoral Dissertation Reading Committee Form (pdf)

Submitted by the student to initiate the department appointment of a Doctoral Dissertation Reading Committee.

Petition for Doctoral Committee Members (pdf)

This petition is to be submitted to the department to request approval for doctoral dissertation reading committee and/or University oral examination committee members who are not members of the Stanford Academic Council.

Change of Adviser or Reading Committee Member (pdf)

If changes are made in your reading committee, this form must be submitted to your department and approved by the chair.

Doctoral Dissertation Agreement Form (pdf)

This form authorizes online publication of your doctoral dissertation and the separate abstract through ProQuest. It is mandatory for all doctoral dissertation submissions.

Oral Examinations

University Oral Examination Form (pdf)

Once the examiners, date, and time for your oral examination have been established, this form must be submitted to your department for approval two weeks prior to the proposed date of your examination.

Diploma Distribution Form (pdf)

Only use this form if you are submitting a Late Application to Graduate. Students who applied to graduate on time must change their diploma distribution through Axess.

Diploma Name (pdf)

Complete this form if you have special accents or special characters that you want to appear on your diploma name.

Late Application to Graduate for Advanced Degree (pdf)

Only use this form after the Application to Graduate deadline. Prior to the deadline, apply to graduate through Axess. There is a $50 fee for submitting this form after the deadline.

Late Application to Graduate for Bachelor's Degree (pdf)

Only use this form after the Application to Graduate deadline. Prior to the deadline, apply to graduate through Axess. There is a $50 fee for submitting this form after the deadline.

Walkthrough Form (pdf)

Use this form to petition your department or school to allow you to walkthrough a Spring Commencement Ceremony without graduating Spring Quarter.

Withdrawal of Application to Graduate (pdf)

Use this form to withdraw your application to graduate.

Degree Certification Request Form (pdf)

Alumni should complete this form to request an Official Degree Certification. See the Certifications and Verifications web site for further instructions.

Replacement Diploma Form (pdf)

If a diploma has been damaged, destroyed, or permanently lost, use this form to order a replacement diploma.

Application for Auditor or Permit to Attend Status (Non-matriculated only) (pdf)

Non-matriculated students may apply for an auditor status (for those affiliated with Stanford only; no fee) or Permit to Attend Status ($2760 fee) in order to audit courses.
